Responsible for providing skilled nursing services to patients in their homes under the direction of a physician. Must assess, plan, implement, document, evaluate and teach patients needs and care. Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.
Provides professional skilled nursing care to patients which includes initial nursing assessment and assessing patient mentally and physically on each visit and reporting to physician when appropriate.
Develops plan of care in collaboration with physicians orders and establishes realistic and measurable goals, updating goals when necessary. Evaluates effectiveness of the care plan frequently.
Performs procedures and treatments with proper techniques and has adequate knowledge of medications actions and effects. Complies with standard nursing policies and procedures. Uses equipment properly to give treatments. Practices cost containment with time and supplies. Supervises LPN and aide as applicable.
Instructs and explains plan of care to patient and family, teaching as needed to encourage self-care. Uses additional services and community resources as appropriate. Plans and prepares patient and family for discharge.
Consults with Physician, patient, caregiver, and other health care professionals regarding coordination of treatment, goal development and response to treatment. Reports on patient at patient care conferences. Identifies need for other disciplines.
Documents accurately and timely patient treatments, care, evaluation, and progress, and communicates with other staff members.
Participates in Quality Improvement activities, inservices, patient care conferences, workshops and seminars. Assumes responsibility for continuing education and maintaining current knowledge.
Assess home situation for possible patient abuse.
Assists with orientation and training of other staff members.
Participates in weekend, holiday and on-call 24-hour call schedule. Position Qualifications:

- We require a nursing degree from an accredited school.
- You'll also need a current RN license in our state.
- Basic life support training should be completed in 90 days with advanced training required at certain intervals.
